Utilizing Point of Care Ultrasound to Evaluate for Small Bowel Obstruction in Emergency Department Patients

چکیده
Small bowel obstruction (SBO) is a common diagnosis made in the Emergency Department (ED) representing about 15% of hospital admissions for acute abdominal complaints. We sought to investigate if bedside ultrasound as performed by ED physicians reliable test diagnose and rule out SBO. This was prospective cohort study nonconsecutive patients who presented an academic, suburban with census approximately 100,000 per year between November 2018 May 2019. Patients history prior surgeries nausea and/or vomiting had CT Abdomen Pelvis PO contrast ordered their provider were consented ultrasound. Interpretation physician performing at bedside. The identified largest loops each four quadrants measured from wall wall. A diameter greater than 2.5 cm considered positive dilated probable obstruction. At completion entered interpretation result which later reviewed fellowship-trained physician. There 101 included study. Study personnel accurate 92% cases. Overall sensitivity point care (POCUS) SBO 90% (72.7 97.8) specificity (82.7 96.9). negative likelihood ratios 10.76 (4.95 23.38) 0.11 (0.04 0.33). Given that our demonstrated low ratio, those probability on could have examination be sufficient this disease without requiring undergo further imaging.

Point-of-care ultrasonography for the diagnosis of small bowel obstruction in the emergency department.
Small bowel obstruction (SBO) is a common cause of acute abdominal pain presenting to the emergency department (ED). Although the literature is limited, point-of-care ultrasonography (POCUS) has been found to have superior diagnostic accuracy for SBO compared to plain radiography; however, it is rarely used in North America for this. We present the case of a middle-aged man who presented with a...

متن کاملEmergency department patients with small bowel obstruction: What is the anticipated clinical course?
BACKGROUND Emergency physicians (EPs) often care for patients with acute small bowel obstruction. While some patients require exploratory laparotomy, others are managed successfully with supportive care. We aimed to determine features that predict the need for operative management in emergency department (ED) patients with small bowel obstruction (SBO). METHODS We performed a retrospective ch...
متن کاملBedside ultrasonography for the detection of small bowel obstruction in the emergency department.
BACKGROUND Plain film radiography (x-ray) is often the initial study in patients with suspected small bowel obstruction (SBO) to expedite patient care. OBJECTIVE To compare bedside ultrasonography (US) and x-ray for the detection of SBO. METHODS This was a prospective study using a convenience sample of patients presenting to the emergency department (ED) with abdominal pain, vomiting, or o...
